For sensitive health information, we require:

1. Direct provision by your treating practitioner
   (We do not accept sensitive data from non-clinical sources)
2. Documented clinical relevance
   (Your clinician must specify why this information is medically necessary for your referral)
3. Compliance with privacy laws
   (Collection is permitted only under Privacy Act 1988 (Cth) Section 16B or equivalent WA health legislation)
